• What do you see? An unsafe condition or an unsafe act? If you said both then you're right. The unsafe act (or more specifically the "failure to act") was caused by the person or persons who saw the spill and failed to clean it up or cover with pads, and who then left an unsafe condition in the work place.
• If you drop, spill or leak (from your equipment) any liquid on the ground the best response is the one you learned from your kindergarten teacher your first day in school: "If you made the mess – you clean it up". Great words to live by.

Don't feel too smug if the picture is not from your location this has probably been your location in the past and it could be in the future. Don't make it so!
